Referrals

Policy

Referrals will be accepted and encouraged from other service providers and support agencies. Documentation of referral services will be kept up to date in the local branch Resource Folder.

Referrals will be in consultation with the client, then the referral agency and client. All actions and casework will be fully documented and kept on the clients file. Referral and case management will be applied and reviewed in accordance with the formal support agreements between Housing Plus and the Support Provider and client.

Any contact with a referral/support service may only occur if an authority had been signed by the client which must be kept on the clients file. Clients must be told they may revoke this authority at any time by notifying the Tenancy Officer. Staff must be aware at all times that our core business is housing management and to be mindful of the limitations of service and staff.
